Amphion Theatre614 Ninth Avenue (between 43rd and 44th streets)
The Amphion was a 578-seat movie theatre that operated from 1919 to sometime in the 1930s. It was not a terribly notable space and although this is the address, I'm not certain that this is the same building, although the architectural detail would indicate that it is of the same era. (reference)
